This is similar in nature to non-bulk billing GPs or visiting a dentist when there is an out of pocket cost that your private health insurance does not cover.

Health care providers, including surgical assistants, must provide the patient with an ‘Informed Financial Consent’ documentation prior to their treatment. This form outlines the ‘out of pocket’ costs that the patient can expect as part of their treatment, to avoid any nasty surprises.
